Cannot add, edit or change Page Size with H and V Series Windows Drivers under Windows 2000 because the New button is inactive and grayed out.
Bug in Windows 2000. The DM_USER_DEFAULT flag is missing from the DOCUMENTPROPERTYHEADER. Described in detail in MSDN article #324439.

Fixed in Service Pack 4 as mentioned in MSDN article #327194 Upgrade to Windows 2000 service Pack 4.
