Running EASYLABEL under Windows NT can sometimes cause certain buttons or icons to change to black boxes or squares.

The display driver is setup incorrectly. From the Settings Tab in the Display Properties in Windows NT, change the number of Colors from 256 to a higher choice like Higher Color or True Color.
